On Your First Visit

Making an Appointment

To make an appointment please call our office at (731) 286-2801 or toll-free (866) 591-4362 (IDOC). Our receptionist will be happy to arrange a suitable appointment time for you. Patients are seen as close to appointment time as possible.

Walk-in emergencies will take priority.

Forms and Information

When you come for your scheduled appointment, you will be asked to fill out approximately 6 pages of pertinent information. These forms may be downloaded and filled out at home prior to your visit.

You will be asked to bring all your insurance cards, your driver’s license, a list of medications you are currently taking, and your glasses. If we can get your insurance information prior to your visit, then verification of benefits can be checked ahead of time to expedite your check-in process.

Your Appointment

It is always a good idea to bring ONE person with you since we might dilate your eyes and you would be unable to drive yourself. Please allow approximately two hours for your routine eye exam experience. There may be additional office time needed to have any special testing or procedures.

After registering upon arrival, a technician will call you to the preliminary testing area. This is where your initial exam will start. Your medical history will be obtained and preliminary testing will be performed to prepare you for your examination by the doctor.

If you are unable to make your scheduled appointment time, we ask that you notify us at least 24 hours in advance. This will allow us to give your appointment time to someone else.

Treatment Clearance Information

If you are being treated for certain conditions, we'll need clearance from the doctor doing that treatment before we can treat you. If any of the following pertain to you, please let us know.

  • Major surgery < 1 month
  • Recently diagnosed diabetes < 3 months
  • Heart attack < 6 months
  • Stroke < 6 months
  • Heart surgery < 1 year

Frequently Asked Questions

  • How long will I be there for the examination?

We like to allow one to two hours.

  • Do I need a driver?

A driver is required after any surgical procedure. If it is a routine exam or yearly examination, a driver is highly recommended.

  • Will you dilate my eyes?

Depending on what kind of problem you are experiencing or if it’s a routine or yearly exam, we will. Dr. Cape feels a dilated exam offers a more accurate and detailed view of the eye as opposed to non-dilated exams.

  • Will you file my insurance?

Yes, but you will be responsible for paying your copay, deductibles, and any outstanding balances prior to being seen by the doctor.

  • May I bring someone with me?

Yes, we do advise you to bring ONE person with you to serve as a driver. This also proves beneficial to have another person hear what the doctor instructs.

  • Where do you do the surgery?

We will be doing your surgery in our own surgery center at the same address as the office.

  • How long does my surgery last?

It depends on what kind of surgery you are having, but usually, the entire surgery process is about two to three hours.

  • How long has Dr. Cape been in Dyersburg?

    Since June of 1993.

  • Is your office wheelchair accessible?

Yes, our office is wheelchair friendly.

  • Do you sell glasses?

No, you will be given your glasses prescription and may take it to the place of your choice.
